Kinder1 - Grade 10
Requirements
- Form 138 (Card)
- Certificate of Good Moral Character
- 2x2 and 1x1 picture (1 copy each)
- Birth Certificate (PSA)(Photocopy)
- JHS Certificate of Completion
Enrollment Procedure
Step 1
Present your credentials to the admitting officer.
Step 2
Proceed to the Principal’s Office for Evaluation of credentials, Interview and Approval
Step 3
Secure and Accomplish Official Registration Form. Return the accomplished form to the Registration Officer for final confirmation.
Step 4
Submit the Accomplished Registration Form to the Accounting Department / Cashier for payment of fees.
Step 5
Proceed to the Registrar's Office for encoding and secure the student’s copy of the official Registration Form.
Step 6
Proceed to the Official Photographer for ID Picture.
Step 7
Proceed to the Library Staff for the application of Library Card.
Step 8
Proceed to the School Clinic for Medical and Dental for check -up.
Senior High School
Requirements
- Form 138 (Card)

Enrollment Procedure
Grade 11
Step 1
Fill out a Personal Data Sheet (PDS) form.
Step 2
Present and submit the credentials for evaluation.
Step 3
Proceed to the Principal’s Office for interview and filling out of Registration Form signed by the Principal.
Step 4
Proceed to the Accounting Office for assessment and payment of fees.
Step 5
Proceed to Registrar’s Office for encoding and secure the student’s copy of the official Registration Form.
Step 6
Proceed to the Official Photographer for school I.D. picture.
Step 7
Proceed to Library for application of Library Card.
Step 8
Proceed to the School Clinic for Medical and Dental Check-up.
Grade 12
Step 1
Present the Report Card and secure registration form from the Assistant Pricipal's Office
Step 2
Fill out the registration form signed and approved by the Assistant Principal
Step 3
Proceed to the Accounting Office for assessment and payment of fees.
Step 4
Proceed to Registrar’s Office for encoding. Secure the student’s copy of the official Registration Form.
Step 5
Proceed to the Official Photographer for school I.D. picture.
Step 6
Proceed to the Library for application of Library Card.
College
A student who graduates from the secondary level, a student who qualified in the Philippine Educational Placement Test (PEPT) for entrance to college and a student who passed the Alternative Learning System (ALS) examination given by the DepEd shall be eligible for admission to any degree program of the College.
A graduate or a transferee from a foreign secondary school should present valid credentials from CHED or DepEd and Department of Foreign Affairs.
First Year Undergraduate
- Form 138 (Original)
- Certificate of Good Moral Character preferably signed by the Principal or Guidance Counselor (Original)
- Must pass the entrance examination set by the school
- Birth Certificate (PSA/NSO) Original & Photocopy
- PSA/NSO Marriage Certificate (for female married students)
- 2”x2” ID pictures (2 copies)
Transferees
- Original Transcript of Record (TOR), Grade Certificate,
- Honorable Dismissal bearing School Seal and Signature by the Registrar
- Certificate of Good Moral Character preferably from the Dean or Guidance Counselor (original)
- Marriage Certificate for married enrollees( photocopy only)
- Must pass the entrance examination set by the school
- Two (2) copies 2” x 2” ID pictures
- Birth Certificate (PSA/NSO) Original & Photocopy
Second Course and Continuing Professional Education (CPE)
Number of Units Required
- 18 units
(with TOR, can take the LET but no Certificate of Completion) - 21 units
(with TOR, can take the LET but no Certificate of Completion) - 30 units
(requires Practice Teaching, can take the LET, with TOR and Certificate of Completion)
- 18 units
Admission Requirements
- Must be a graduate of any Bachelor's Degree Program
- Must have the original and photocopy of the Transcript of Record with Special Order
- Must have a copy of the college diploma
- Must take and pass the entrance examination
- Must have a copy of PSA birth certificate
- Must have a certificate of good moral character
- Must have a copy of the honorable dismissal
- Must submit two (2) copies 2” x 2” ID pictures
Graduate Studies
Requirements
- Original Transcript of Records with Special Order (S.O)
- Permit to Study ( for Government Employee)
- Photocopy of Birth Certificate
- Photocopy of Marriage Certificate (For Female Married Applicants)
- Two (2) copies 2X2 Pictures
